چکیده

2. ained into the right ventricle (Fig. 1A). The underdeveloped ht descending coronary artery presented another coronary tery fistula draining into the right ventricle (Fig. 1C). Both tula paths were occluded by deploying 10 coils via radial cess (Figs. 1B and D). Final results were good with no mplications. During her check-up after 8 weeks, the patient reported a ear improvement in functional class, having presented no rther symptoms of angina despite receiving no anti-angina ugs other than 100 mg losartan/day for high blood pressure ntrol for more than 6 years. She underwent control exercise sting, completing the Bruce protocol stage 3 and reaching 96% MHR with neither electrical nor clinical changes. During a cond stress echocardiograph test, she reached MHR in the aximum stage of 40 mg/kg/min without presenting electrical echocardiographic changes. A congenital coronary artery fistula is an abnormal commucation between an epicardial coronary artery and a cardiac amber or vascular structure located near the heart: 40% to the ht ventricle, 25% right atrium, 15% to 20% pulmonary artery and coronary sinus. It is found in approximately 0.10% to 0.15% of ronary angiographies and most frequently affects the right scending coronary artery (60%). It is usually found by chance ith no identifiable associated symptoms, but symptoms do pear when the shunt is significant, fundamentally variable grees of ischemia and of heart failure. Dual coronary artery fistulas affecting both coronary territories e extremely rare (5% of the total) and more frequently present enomena of secondary coronary ischemia than of significant ft-right shunt with phenomena of ‘‘coronary steal’’. The rcutaneous approach and coil deployment–which is less 3.

Neo Left Main Channel Creation Using Double Stenting Alongside a Sapien 3 Aortic Valve Bioprosthesis for Left Main Coronary Obstruction Following Valve-in-Valve Transcatheter Aortic Valve Replacement: A Case Report With Review of Literature

Transcatheter aortic valve replacement in the setting of failed surgical bioprosthesis (valve-in-valve) is a valuable option for patients with bioprosthetic aortic stenosis or regurgitation who are deemed high risk for repeat open heart surgery. Although the procedure is successful with proper preprocedural assessment, instances of left main (LM) coronary artery ostium obstruction have been doc...

Radiation Therapy-Induced Cardiovascular Disease Treated by a Percutaneous Approach

We report the case of a 51-year-old woman, treated with radiotherapy at the age of two years, for a pulmonary sarcoma. Subsequently she developed severe aortic stenosis and bilateral ostial coronary artery disease, symptomatic for dyspnea (NYHA III functional class). Due to the prohibitive surgical risk, she underwent successful stenting in the right coronary artery and left main ostia with dru...

Coronary stenting for iatrogenic stenosis of the left main coronary artery post-aortic valve replacement: an alternative treatment?

Iatrogenic coronary ostial stenosis after aortic valve replacement is a rare, life-threatening complication, which may follow implantation of either a mechanical or a biological prosthesis. Historically, this condition has been treated by urgent coronary bypass surgery but is associated with high morbidity and mortality, due to the hazards of early repeat sternotomy. We report a case of iatroge...

Pathology of transcatheter valve therapy.

OBJECTIVES This study sought to report on the pathology of transcatheter aortic valves explanted at early and late time points after transcatheter aortic valve implantation. BACKGROUND Information on pathological findings following transcatheter aortic valve implantation is scarce, particularly late after transcatheter aortic valve implantation. METHODS This study included 20 patients (13 m...

Emergent Chimney Stent to Treat Left Main Occlusion Following Valve-In-Valve Transfemoral Aortic Implantation Chimney Stent Following Valve-In-Valve TAVI

Implantation of transcatheter valves inside degenerated surgical bioprosthesis is nowadays a very good alternative to redo a new cardiac surgery especially in high risk patients. Although procedural success is achieved in most of the patients, these procedures are associated with few potential risks such as coronary occlusion. It is very important to perform an exhaustive study prior to the val...
